IMPORTANT NEWS
We would like to inform you about some important updates at NPMC as part of our ongoing commitment to improving patient access and the quality of care we provide.
1. Changes to Administrative Forms
Starting 1st April 2025, we will no longer be accepting forms that require a significant amount of time from our GPs to complete. These forms can include medical reports for insurance claims, patient disability assessments, and other forms that involve extensive administrative work. By ceasing the completion of these forms, we aim to free up valuable time for our GPs to focus on direct patient care, reducing waiting times and improving access to appointments.
2. Forms We No Longer Accept
From 1st April 2025, the practice will no longer be able to complete the following forms:
- DWP Forms
- Firearm Forms
- Cancellation Forms (e.g. holiday cancellations, concert cancellations, etc.)
If you submit any of these forms, we will return them to you with the following message:
- DWP Forms: “We have received a request from DWP. Please contact DWP directly for further assistance.
- Firearm Forms: “We have received your request to complete a firearms form. Please contact the provider or ShootCert for further assistance.
- Patient Forms: We have returned it to you via post. Please contact your insurance company for further advice.
For Firearm Forms, patients can reach out to ShootCert Limited for assistance:
