Carers Advice

Are You a Carer?

If you are please let us know via the online form below – we may be able to help you.

A carer is someone who provides care for a relative, friend or neighbour. The care can be of a personal or domestic nature, or maybe monitoring and prompting to ensure the “cared for” comes to no harm. Some carers have a substantial responsibility and as a result may neglect their own health and become every tired and as a consequence put their caring role at risk.

At the Medical Centre we try to know who our carers are, so we can offer them advice, support and encouragement.

Register a Carer Form

It is important that we know if you are a carer so that we can make sure you receive information, services and the help that is available. If you are a carer please complete this form.
