Patient Navigator
Newport Pagnell Medical Centre is looking to recruit patient navigators to join our team. You will be required to work 21 hours per week between Monday to Friday over four days and be available on the fifth day to provide cover if needed. You will also be required to cover Saturday morning shifts on a rota basis.
We offer a comprehensive training programme to all our patient navigators where you will initially receive 1-1 training, followed by ongoing support from your trainer and team leaders.
There is more to this role than answering the phone and making appointments. In this essential front-line role at the practice, our patient navigators help patients to access the right services, assist with keeping health records accurate and up-to-date, and ensure that appointment bookings are well-managed.
The main responsibilities of the role include:
- Greet patients, doctors & visitors in a polite & friendly manner
- Answer the telephone promptly & courteously
- Make appointments using SystmOne
- Administer the procedure for repeat prescriptions
- Open & sort all the Medical Centre post
- Ensure the smooth running surgeries and clinics
- Arrange hospital appointments
- Receive specimens ready for pathology laboratory collection
- Deal with general enquiries helpfully & efficiently
- Collate, sort & deal with paperwork from GPs & other practice staff
- Inform patients of test results
- Support colleagues in maintaining an efficient & friendly team atmosphere

Community Staff Nurse

We are seeking a Community Staff Nurse for our Community Nursing Team. You will be based at Newport Pagnell Medical Centre. You will be a registered nurse 1st level, be a car driver and have community experience, however training will be offered.
The successful candidate will be employed by Newport Pagnell Medical Centre and work alongside a forward-thinking, supportive integrated Clinical team (practice nurses, district nurses, specialist nurses and community matrons), managed by a Lead Nurse.

Practice Nurse
14.5 hours Salary dependent on experience.
Newport Pagnell Medical Centre is recruiting a practice nurse on a part-time basis, working 14.5 hours per week. We are keen ideally, to hear from those with asthma and cardiovascular disease experience.
The practice holds Saturday morning clinics 0800-1230 working alongside a duty GP – these are managed by the nursing team on a rota basis and will be additional to the contracted hours.
In this role, you will be responsible for the delivery of general practice nursing services to the practice population.
Key responsibilities:
- Assess, plan, develop, implement and evaluate programmes to promote health and wellbeing.
- Implement and evaluate individual treatment plans for patients with a known long term condition.
- Proactively identify and manage as appropriate, treatment plans for patients at risk of developing a long term condition.
- Prioritise health problems and intervene appropriately to assist the patient in complex, urgent or emergency situations.
- Support patients to adopt health promotion strategies that encourage patients to live healthily.
- Provide information and advice on prescribed, or over the counter medication on medication regimens, side effects and interactions.
- Support and manage health needs of women presenting for cervical cytology consultations.
- Recognise, assess and refer patients presenting with mental health needs. Implement and participate in vaccination and immunisation programmes for both adults and children.

Part-time Minor Illness Nurse (20 hours)
Newport Pagnell Medical Centre has a fantastic opportunity for an experienced minor illness nurse to join our urgent care team on a part-time basis, for 20 hours per week, 0800-1830 Monday to Friday and the salary for these hours is £26,000.
The practice nursing team forms part of our integrated nursing team working alongside our community nursing team, community matrons, urgent care team and specialist nursing team.
The main responsibilities of the role include:
- Working closely with GP colleagues, the minor illness nurse will deliver minor illness clinics.
- Providing assessment, treatment, screening, health education services and advice to patients.
- Long term condition case management.
- Dealing with minor illness and injury.
- Diagnosing, prescribing and referring.
About the Candidate
All applicants should have the following qualifications, registrations, skills and experience:
- A relevant nursing degree.
- Current NMC registration with an active PIN.
- Run autonomous minor illness clinics.
- Experience and competence in managing a general practice caseload.
- Prescribe products to meet the needs of patients within the Practice population.

Part-time Pharmacist (18 hours per week)
Newport Pagnell Medical Centre is looking to welcome pharmacy technician into our busy practice, to play a full and active role within our multi-disciplinary team. The role is offered on apart time (18 hours) basis and the salary for these hours is £14,040pa.
Main responsibilities of the role will include:
- Support the multidisciplinary team to ensure efficient medicines optimisation procedures are followed.
- Stream general prescription requests and work with the reception and acute script team, so as to allow GPs and clinical pharmacists to review more clinically complex requests.
- Assist patients, colleagues and external healthcare professionals with responses to medication queries.
- With support of the GP and Pharmacy Team, action medicine changes from clinical letters and specialist requests, in line with local formulary and prescribing guidelines.
- Provide training and support on the legal, safe and secure handling of medicines, including the implementation and increased use of the Electronic Prescription Service (EPS).
- Implement efficient ordering and return processes and reduce medication wastage, liaising with patients as needed.
- Support the implementation of national prescribing policies and guidance within GP practices, care homes and other primary care settings.
- Reauthorise and synchronise repeat prescriptions appropriately, ensuring patients have had, or are invited to, medication monitoring blood tests and clinical reviews.
- Help to organise and run various systems for the delivery of QOF and locally commissioned enhanced services.
- Support cost-effective prescribing through switching medicines to recommended, cost-effective alternatives
- Review and develop policies in line with CQC compliance.
- Develop relationships with other pharmacy professionals and members of the multi-disciplinary team including primary care, community pharmacy, secondary care and mental health.
- Contribute to the delivery of effective daily processes for the transfer of care between hospitals and GPs including effective signposting, record updates and relay communication.
About the candidate:
The successful candidate will be:
- Be a registered pharmacy technician with the GPhC.
- Hold a Diploma Level 3 in pharmacy services, or equivalent.
- Have strong experience in either a community pharmacy or a primary care setting.
- Be confident in analysing and presenting information to health professionals.
- Have experience of implementing and promoting cost-effective strategies.
- Have the ability to interpret complex clinical information and present it in suitable formats, to a variety of people from professional and non-professional backgrounds.
