If you require travel vaccinations, please complete a travel questionnaire, available below, print and post to NPMC for the attention of the Practice Nurse Team or attach to an email and send to blmkicb.npmcmail@nhs.net at least 6 weeks before travel if possible.   This service is for NPMC NHS patients only.   Not all travel vaccinations are included in the services provided by the NHS so you may also be referred to our private travel clinic.

If you are travelling abroad please make sure you contact us in plenty of time to arrange any vaccinations that may be necessary. To help the Travel Nurses assess your travel needs it is important that they are in receipt of the assessment form before your appointment. Please be aware that we require at least 6 weeks notice before you travel to allow time for your vaccinations.

Personal Medical History
