Patient Navigator positions

We are looking for well motivated, flexible employees to join our busy reception team who strive to ensure our patients receive the best possible service from the Practice.

You will be responsible for all reception duties, providing a high standard of face to face and telephone care for patients, staff and visitors.

You need excellent customer service and communication skills and a good knowledge of Windows-based computer systems.  Experience of working with SystmOne would be an advantage.

Each successful candidate will undertake a 4 month supervised training programme where you will learn the various aspects of the role working alongside a supportive Reception Team.

We have part time vacancies (21 hours per week) - working a combination of morning and afternoon shifts of either 5/5.5 hours over four days with availability on the fifth day to cover for your colleagues.  You will also be expected to work a Saturday morning shift on a rota basis.  

The starting hourly rate will be £9.50 per hour, but this will increase to £10.00 once training has been successfully completed.

Community Staff Nurse

We are seeking a Community Staff Nurse for our Community Nursing Team.  You will be based at Newport Pagnell Medical Centre but support two GP practices.  You will be a registered nurse 1st level, be a car driver and have community experience, however, training will be offered. 

The successful candidate will be employed by Newport Pagnell Medical Centre and work alongside a forward thinking, supporting integrated nursing team (practice nurses, district nurses, specialist nurses and community matrons), managed by a Lead Nurse.

The post offers the opportunity to provide innovative district nursing services focused on caring for housebound patients.  Our approach is holistic, and evidence based in the assessment, implementation and evaluation of practice.

Community Healthcare Assistant

Ideally you will have experience of working in the community, but training can be offered for the right person.

We are seeking a Community Healthcare Assistant for our Community Nursing Team.  You will be based at Newport Pagnell Medical Centre, but support 2 GP practices.  You will provide planned nursing care to patients and their families in their own homes or other setting as appropriate and for the majority of the time, the postholder will work independently.  

Both part time/full time hours will be considered for the right candidate.

Community Nurse Associate

Would you like to work within a supportive team?  Would you like the opportunity to care for people in their own home?  We are looking to appoint a Nursing Associate who will offer high quality care to patients in the community setting working within our Community Nursing Team.
