Community Matron

Part time - 24 hours per week

We are looking for a Community Matron to join our team.  You will be based at Newport Pagnell Medical Centre supporting 2 GP practices managing the Community Matron caseload.

Our Community Matron is given information of all admissions to A&E or the Walk in Centre for over 75 year olds.  As part of the role, The Community Matron will make contact with the patients and act as necessary.

The successful candidate will have experience of:

  • Ideally have a nursing background with experience of carrying out health checks.
  • Signposting to services in the Milton Keynes area.
  • Case management
  • Supervising team members
  • Knowledge of dementia
  • Safeguarding
  • Dealing with Power of Attorney, attendance forms, memory assessments.
  • Submitting patient attendance data in line with contract requirements.  
  • Acting as an advocate for patients on the caseload.
  • Assessing the needs of a patient with regard to aids within the home, eg. walking aids, bathroom equipment

The successful candidate will be employed by Newport Pagnell Medical Centre and work alongside a forward thinking, supportive nursing team (practice nurses, district nurses, specialist nurses and community matrons), managed by a Lead Nurse.

The successful candidate will need to develop close links with the Specialist Memory Service and the staff working at The Mead Centre and Brooklands within Newport Pagnell who offer support for carers in the area.

Part Time Bookkeeper

We are looking for a part time competent bookkeeper who must have experience of both SAGE processing and Excel to work with us here at Newport Pagnell Medical Centre.

Newport Pagnell Medical Centre is a forward thinking practice with 22,000+ patients and additional services that we offer to patients across the whole of the Milton Keynes area.  Our Accounts Team support the whole practice in a range of tasks which makes the role both interesting and rewarding and would give the right candidate the opportunity to use all of their skills.

Ideally we are looking for candidates who have good experience of the following skills:

  • SAGE processing - sales, purchases and nominal.
  • Excel
  • Monthly reconciliations
  • General administration

Patient Navigator positions

We are looking for well motivated, flexible employees to join our busy reception team who strive to ensure our patients receive the best possible service from the Practice.

You will be responsible for all reception duties, providing a high standard of face to face and telephone care for patients, staff and visitors.

You need excellent customer service and communication skills and a good knowledge of Windows-based computer systems.  Experience of working with SystmOne would be an advantage.

Each successful candidate will undertake a 4 month supervised training programme where you will learn the various aspects of the role working alongside a supportive Reception Team.

We have part time vacancies (21 hours per week) - working a combination of morning and afternoon shifts of either 5/5.5 hours over four days with availability on the fifth day to cover for your colleagues.  You will also be expected to work a Saturday morning shift on a rota basis.  

The full time vacancies are 37.5 hours per week (7.5 hour days) between the hours of 0800 and 1830 Mon to Fri.  

The starting hourly rate will be £9.50 per hour, but this will increase to £10.00 once training has been successfully completed.
