Patient Navigator positions

We are looking for well motivated, flexible employees to join our busy reception team who strive to ensure our patients receive the best possible service from the Practice.

You will be responsible for all reception duties, providing a high standard of face to face and telephone care for patients, staff and visitors.

You need excellent customer service and communication skills and a good knowledge of Windows-based computer systems.  Experience of working with SystmOne would be an advantage.

Each successful candidate will undertake a 4 month supervised training programme where you will learn the various aspects of the role working alongside a supportive Reception Team.

Working 21 hours over four days - a combination of morning and afternoon shifts between the hours of 0800 and 1830 Mon to Fri.  You will be expected to be flexible and work a 5th day when needed and you will work Saturday mornings on a rota.  Location - Newport Pagnell Medical Centre, but you may be required to work shifts at our NPMC@Willen site in the future.

The starting hourly rate will be £9 per hour, but this will increase to £9.50 once training has been successfully completed.

Community Staff Nurse - full time, but part time considered

Ideally you will be community experience, but training can be offered for the right candidate.

We are seeking a Community Staff Nurse for our Community Nursing Team.  You will be based in Newport Pagnell Medical Centre but support 2 GP practices.  You will be a registered nurse 1st level, be a car driver and have community experience, however, training can be offered.

The successful candidate will be employed by Newport Pagnell Medical Centre and work alongside a forward thinking, supporting integrated nursing team (practice nurses, district nurses, specialist nurses and community matrons), managed by a lead nurse.

Diabetes Nurse Hours - 15 hours per week

Minor Illness Nurse

We are looking to recruit a Minor Illness Nurse to work closely with our GP colleagues.  The successful candidate will deliver Minor Illness Clinics as part of the Urgent Care Team, providing assessment, treatment, screening, health education services and advice to patients.  You will have experience and competence in managing a General Practice caseload including chronic case management, dealing with minor illness and injury and experience in diagnosing, prescribing and referring.

Health Care Assistant

We are looking for Health Care Assistants to join our Practice Nursing Team.  You will be based at Newport Pagnell Medical Centre, supporting 2 GP practices.  Full time and part time both considered.

The successful candidate will have previous experience of working as a Health Care Assistant, have good communication skills and be computer literate.  A HCA Diplome or Care Certificate is preferable but not essential.
